[ Overview ]

The Emergency Department Care Coordinator (EDCC) works in collaboration with the Emergency Department Physicians, mid-level providers, nurses and appropriate departmental managers/directors.  The core purpose of the EDCC is to work to connect ED treated and released patients to CentraState Medical Center outpatient services, programs and PCP providers.

[ Responsibilities ]
  • Works with the ED team to collaboratively formulate appropriate and realistic discharge plans
  • Uses knowledge of current information on CentraState’s ambulatory and out-patient resources and refers patients to the resources needed to prevent ED visits and hospital admissions
  • Understands other community agencies and works with these agencies to identify potential resources for resolving patient’s, psychosocial, financial, transportations or barriers to accessing ambulatory and out-patient care
  • Communicate with insurance representatives to enable financial coverage of post-acute services

 

[ Qualifications ]

Education:

Associates Degree preferred

 

Experience:

Experience in Population Care Coordination, management of transitions in care and ambulatory follow up care preferred

Experience with medical terminology

Experience with payers including Medicare, Medicaid, self-pay, charity care and commercial insurance products, with a firm understanding of eligibility, coverage, prior authorizations, co-insurance, deductibles, and other related issues

Knowledge of the community, its transportation system, its services for the disabled, its nursing homes, its clinics, its hospitals, and their departments such as emergency rooms, radiology facilities, etc.

 

Required Knowledge, Skills and Abilities:

Extensive familiarity with CentraState Medical Center programs; Critical thinking and problem solving skills; Ability to effectively utilize resources to meet patient needs; a willingness and ability to break down barriers to care for patients; Strong effective communication skills and enthusiasm for the work

[ Physical Demands ]

Working Conditions:

Works in a normal hospital Emergency Department environment with frequent exposure to sick or injured patients. Works with outpatient populations, treated and released ED patients.

 

Exposure:

Daily potential for exposure to blood and/or body fluids.
