Overview

Develops, plans, organizes and implements high quality community wellness health education programs and events with guidance from the Community Wellness Manager. 

Responsibilities

  • Works with population health manager to identify target populations for health management services.
  • Performs outbound telephone calls to targeted healthcare consumers. Counsels consumers in person and telephonically on individual health risks and guides them to take necessary action to prevent migration to a higher risk level.
  • Refers consumers to enroll in community wellness health education programs or to consider outpatient services such as cardiac or physical rehabilitation.
  • Uses telemedicine tools when appropriate to monitor high or rising risk patients. Performs outreach counseling to help direct the consumer to the appropriate level of care. Collaborates with other PCPs and other healthcare providers to assure optimal care.
  • Coordinates and conducts health screenings in alignment with population health goals. Informs screening participants of their results, educates participants on their health risks and refers them to appropriate care providers for follow up as needed.
  • Studies the results of community needs assessments, health risk appraisals, medical claims trends and other data to identify areas of greatest need for health coaching and community wellness program planning.
  • Establishes effective strategies for teaching using various methodologies appropriate to age and learning ability of the client(s). Investigates and implements evidence-based approaches to behavior modification. Revises course content and/or teaching methodologies as warranted.
  • Works with others to apply for grant funding and community awards.
  • Represents department to external agencies.
Qualifications

  • BSN Preferred.
  • Current NJ RN license required.
  • Three years of related experience. 
  • Health Coaching experience preferred
  • Knowledge of specific disease and lifestyle related topics 
  • NOTE – this position is not a remote position – on-site position

Compensation Range: $44.00 – $74.80 per hour

 

The compensation above reflects the established range from CentraState Healthcare System (CSHS) for this position at the time the job was posted. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training.  As such, team member compensation may fall outside this range. 

 

Additionally, the compensation range reflects base salary and does not include extra shift rates or incentives tied to quality, productivity, etc., as applicable.
