The Statesir Cancer Center at CentraState Medical Center is seeking an Oncology dedicated Social Worker. The Cancer Care Center Social Worker provides psycho-social support counseling and advocacy services to assigned patients and families as needed.

Responsibilities

  • Provides counseling for patients and families
  • Performs psychosocial assessments
  • Attends and participates in multidisciplinary Tumor Boards
  • Offers and facilitates support groups
  • Assists with completion of charitable applications / community assist / patient assistance programs
  • Assists with Medication applications
  • Evaluates patient for Medicare/Medicaid/ Charity Care eligibility
  • Identifies community resources. Provides patient/family with information on Community Services.
  • Provides transportation resources and other community resources
  • Coordinates Community Assistance Programs
  • Educates on hospice and refers to the Palliative Care team as necessary
  • Assists with end-of-life decision making in cooperation with the Palliative care team
  • Provides bereavement follow-up
  • Provides considerate respectful care focused upon the Residents’ individual needs.
  • Affirms the Resident’s right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law.
  • Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the Residents cared for.
  • Demonstrates an understanding of the range of care needed based on the age of the patients cared for.
  • Demonstrates knowledge of growth, development and/or adult health
    throughout the life span as appropriate to the job performed. Successfully completes lifespan education as appropriate.
  • Reviews admissions and identifies patients meeting with end of life issues. All assessments shall include psycho-social, living situation and available support.
  • Interviews patients and/or family members as appropriate to determine patient needs.
    Identifies situations that require follow up.
  • Provides support to patients and families coping with illness and hospitalization.
  • Makes recommendations regarding needs of patients and suggestions for providing more effective comprehensive care and social work services.
  • Participates in departmental Quality Improvement activities by collecting and documenting quality improvement statistics as required.
  • Documents all assessments, referrals and interventions in the patient’s medical record.
  • Maintains monthly statistics with regard to patients served and completes monthly report.
  • Works collaboratively with Physician, RN and Cancer navigator regarding care management.
  • Provides diversional activity and emotional support as needed.

 

Qualifications

  • Bachelor’s Social Work required
  • BS/BA in Social Work from accredited college or university and internship required
  • MSW Preferred
  • Two (2) Years’ Medical Social Work Experience preferred
  • Certified Social Worker / New Jersey License Required
  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team

Compensation Range: $33.00 per hour – $52.80 per hour.

 

The compensation above reflects the established range from CentraState Healthcare System (CSHS) for this position at the time the job was posted. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training.  As such, team member compensation may fall outside this range. 

 

Additionally, the compensation range reflects base salary and does not include extra shift rates or incentives tied to quality, productivity, etc., as applicable.
